SUMMARY: The Social Service Worker helps clients by assessing their situation; setting goals; obtaining required services. They establish course of action by exploring options; setting goals with client and obtains assistance for client by referring him/her to community resources; arranging for appointments; establishing rapport with other agencies.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs.
Collaborate with other professionals to assess client needs or plan treatments
Advocate for clients or patients to resolve crises.
Intervene in crisis situations to assist clients.
Refer patient, client, or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education.
Refer individuals to educational or work programs.
Refer clients to community or social service programs.
Counsel clients and patients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life.
Counsel clients or patients with substance abuse issues.
Monitor, evaluate, and record client progress per measurable goals described in treatment and care plan.
Monitor clients to evaluate treatment progress.
Maintain client records.
Plan discharge from care facility to home or other care facility.
Modify treatment plans to comply with changes in clients' status.
Modify treatment plans to accommodate client needs.
Identify environmental impediments to client or patient progress through interviews and review of patient records.
Collect information about clients.
Interview clients to gather information about their backgrounds, needs, or progress.
Plan programs to address community health issues.
Complete documentation required by programs or regulations.
Complete & file required record keeping forms/charts upon the resident’s admission, transfer and/or discharge. Chart notes in an informative & descriptive manner that reflects the care provided to the resident, as well as the resident’s response to the care. Perform routine charting duties as required & in accordance with established charting & documentation policies & procedures.
Maintain confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.
Performs other duties as assigned.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill, and/or ability required.

Must be able to read, write, speak, and understand the English language.
Must be able to make independent decisions when circumstances warrant such action.
Must be knowledgeable of social services & procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
Experience in Long Term Care is preferred.
