We are seeking a Patient Navigator to join our growing private OB-GYN practice. In this role, you'll serve as the primary point of contact for patients, ensuring they receive timely, coordinated care from diagnosis through treatment and follow-up.

- 1 remote workday each week
- Up to $25.00 per hour
What You'll Do
As our Patient Navigator, you'll play a vital role in delivering exceptional patient care by:
- Coordinate patient care between providers, specialists, hospitals, imaging centers, and laboratories
- Schedule follow-up appointments, referrals, procedures, diagnostic testing, and surgeries
- Monitor care plans from start to finish, ensuring every patient receives timely follow-up
- Track laboratory, pathology, and imaging results and communicate them promptly to providers and patients
- Educate patients about diagnoses, medications, treatment plans, surgical preparation, and recovery
- Identify and resolve barriers to care such as scheduling conflicts, transportation challenges, insurance issues, or financial concerns
- Conduct outreach to patients who miss appointments or delay recommended treatment
- Coordinate surgical scheduling, authorizations, and pre-operative clearances
- Perform post-operative follow-up and monitor patient recovery
- Maintain accurate documentation within the electronic medical record (EMR)
- Escalate urgent clinical concerns to providers when appropriate
What We're Looking For
Required Qualifications
- Medical Assistant certification or equivalent hands-on healthcare experience
- Minimum of 2 years of experience in one or more of the following:
- Patient Navigation
- Care Coordination
- Case Management
- Women's Health
- OB-GYN
- Medical Office or Clinical Practice
- Experience using Electronic Medical Records (EMR)
- Excellent communication and patient education skills
- Strong organizational skills with the ability to manage multiple priorities
- Detail-oriented with exceptional follow-through
