Nurse Practitioner (NP) / Physician Assistant (PA) – Skilled Nursing Facility
Location: Oneonta, NY
Schedule: 3–4 Days Per Week Onsite + Call Coverage on Scheduled Workdays

Genesis Medical Health is looking for an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our medical team providing primary care to residents of a skilled nursing facility in Oneonta, NY.
This position offers a flexible 3–4 day per week onsite schedule, strong earning potential, and the opportunity to become part of an established medical group focused on providing high-quality, consistent care to geriatric and post-acute patients.
Position Highlights
- $5,000 sign-on bonus

- Wednesday coverage is already in place

- Approximately 25–30 patient encounters per day
- Call coverage on scheduled workdays from 7:00 AM–9:00 PM, with occasional coverage potentially extending until 11:00 PM

Responsibilities
As part of the Genesis Medical Health team, the NP or PA will manage the day-to-day medical needs of residents within the facility, including:
- Providing primary, post-acute, and long-term care
- Completing admissions, follow-up visits, and routine assessments
- Managing chronic medical conditions and acute changes in condition
- Reviewing labs, imaging, medications, and other clinical reports
- Coordinating care with physicians, nursing staff, specialists, families, and facility leadership
- Responding to clinical calls during assigned coverage hours on scheduled workdays
- Providing clinical guidance for urgent changes in condition
- Helping prevent unnecessary hospitalizations through proactive patient management
- Completing documentation and EMR charting accurately and on time
Qualifications
- Active New York Nurse Practitioner or Physician Assistant license
- Available to work 3–4 days per week onsite in Oneonta
- Comfortable providing telephone call coverage on scheduled workdays
- Strong clinical judgment and ability to practice independently
- Experience in skilled nursing, nursing homes, geriatrics, primary care, or internal medicine preferred
- Comfortable managing medically complex and geriatric patients
- Reliable transportation
