Location: Remote (must reside in Indiana)
Schedule: Monday–Friday, 8:00 AM – 5:00 PM ET
Pay: $90,000–$93,000 annually (based on experience)

We are seeking a dedicated and experienced OB Nurse Case Manager to join our team supporting expectant mothers throughout their pregnancy and postpartum journey. This role requires a minimum of three (3) years of Labor & Delivery experience, as you’ll serve as a trusted guide and educator for members navigating pregnancy, delivery, and recovery.
You’ll play a key role in improving maternal outcomes by coordinating care, providing education, and connecting members with the services and resources they need. This is a fast-paced, full-time position that requires strong organization, critical thinking, and compassion.

Manage a caseload of pregnant members, providing telephonic support and care coordination

Educate and guide members through pregnancy, delivery, and postpartum care.

Conduct comprehensive assessments to identify individual needs and develop care plans.

Coordinate medical appointments, referrals, and follow-up care across providers and community resources.

Monitor care plans, evaluate progress, and make adjustments as needed.

Ensure members have access to appropriate services based on their health needs.

Maintain accurate documentation in insurance systems and adhere to compliance standards.

Collaborate with physicians, social workers, and other care team members to optimize member outcomes.
Requirements:

Education: Bachelor’s degree in a health-related field preferred.

Experience: Minimum of 3 years of clinical Labor & Delivery experience (required).

Licensure: Active, unrestricted RN license in Indiana (multi-state licensure preferred).

Certification: Case Management certification preferred.

Strong organization, attention to detail, and time management skills.
