Job Overview
A+P House Calls is a nurse practitioner–owned concierge medical practice providing in-home and telemedicine care to adult patients across Long Island.
We are seeking an experienced and highly capable Adult Nurse Practitioner to join our team. This is an autonomous, field-based role delivering high-quality, patient-centered care directly in patients’ homes, as well as through telehealth.
This position is ideal for a provider who is confident, clinically strong, and thrives in an independent environment.
Responsibilities
- Provide in-home and virtual care to adult patients
- Perform comprehensive assessments, physical exams, and medical decision-making
- Diagnose and manage acute and chronic conditions
- Deliver primary and urgent care services in a home setting when needed
- Develop and implement individualized treatment plans
- Lab and radiology interpretation
- Prescribe medications using ePrescribe platforms
- Maintain accurate documentation using HIPAA-compliant systems
- Conduct telehealth consultations and follow-ups
- Educate patients on treatment plans, medications, and preventive care
- Collaborate with the care team to ensure continuity and quality of care
Qualifications
- Licensed Adult Nurse Practitioner (NP) – Board Certified
- Minimum 5 years of experience as a provider (required)
- Strong background in urgent care, emergency medicine, and/or primary care preferred
- Ability to work independently with strong clinical judgment
- Experience managing a wide range of patient conditions
- Proficiency with:
- ePrescribe / eMAR systems
- HIPAA-compliant platforms
- Must have a reliable vehicle and be willing to travel locally
- Must have laptop for on the road ordering/transcribing/communication
- Strong communication and interpersonal skills
Schedule
- Full-time position
- 6–8 hour workdays, 5 days per week
- Every other weekend
- Flexible scheduling based on patient needs and concierge model
- Includes both in-person visits and telehealth services
