About Us
We are a growing healthcare organization focused on improving care for chronically ill patients through Chronic Care Management (CCM) and Remote Patient Monitoring (RPM). Our remote clinical teams work closely with patients and physician practices to identify concerns, improve care coordination, and help patients better manage their health between office visits.
We are looking for a dependable and experienced Registered Nurse to serve as a Clinical Lead RN supporting our RN Director, Care Coaches, patients, and assigned accounts.
Job Summary
The Clinical Lead RN is a hands-on clinical support position with both patient care and team leadership responsibilities. You will serve as a primary clinical resource for an assigned team of Care Coaches, which may grow to approximately 7-8 team members. When a Care Coach identifies a clinical concern, you will help assess the situation, determine the appropriate next step, and ensure the patient is directed to the appropriate level of care. You will also review and sign care plans, assist with clinical documentation and patient needs, support assigned client accounts, and work closely with our RN Director on day-to-day clinical operations.
This position may also assist the RN Director with coaching, performance, or employee management matters involving assigned team members.
Key Responsibilities
- Serve as the primary clinical resource for assigned Care Coaches and patient accounts.
- Provide day-to-day clinical guidance and support to a team that may grow to approximately 7-8 Care Coaches.
- Review patient concerns escalated by Care Coaches and determine the appropriate course of action.
- Triage clinical concerns and help coordinate appropriate follow-up with the patient's provider, emergency services, or other healthcare resources when indicated.
- Review patient vital signs, symptoms, medical history, medications, and other available clinical information when evaluating escalations.
- Review and sign patient care plans and ensure they are clinically appropriate and properly documented.
- Assist with the development and updating of care plans when RN-level clinical input is needed.
- Support CCM and RPM patients whose needs require RN-level involvement.
- Work closely with the RN Director on clinical workflows, escalations, account needs, and patient care concerns.
- Help ensure Care Coaches follow established clinical, documentation, and escalation processes.
- Provide coaching and clinical guidance to Care Coaches to improve patient care and team performance.
- Assist the RN Director with employee performance or management matters involving assigned team members when needed.
- Communicate appropriately with physician practices regarding patient concerns and care coordination needs.
- Maintain accurate and timely documentation of patient interactions and care coordination activities.
- Meet established productivity, responsiveness, documentation, and quality expectations.
- Maintain patient confidentiality, rights, and dignity in all interactions.
- Perform additional duties as needed to support patient care and program operations.
Qualifications
- Active, unrestricted Registered Nurse (RN) license in your state of residence.
- Minimum of 2 years of nursing experience.
- Must speak Arabic
- Strong clinical judgment and ability to appropriately assess and triage patient concerns in a remote environment.
- Ability to recognize when a patient's condition requires provider notification, urgent evaluation, emergency care, or other intervention.
- Comfortable providing clinical direction and support to non-RN clinical team members.
- Excellent verbal and written communication skills.
- Strong documentation and organizational skills.
- Ability to work independently with minimal supervision.
- Ability to manage multiple priorities and remain responsive throughout the workday.
- Proficient with computers and comfortable working with electronic medical records and remote healthcare platforms.
- Dependable, professional, and accountable.
Preferred Experience
- Experience in Chronic Care Management (CCM) and/or Population Health.
- Experience with Remote Patient Monitoring (RPM).
- Experience in care management or care coordination.
- Experience reviewing or developing patient care plans.
- Previous experience as a lead, preceptor, charge nurse, or clinical resource.
- Experience working with remote healthcare teams.
- Bilingual communication skills are helpful.
Work Schedule
This position requires a commitment to a 40-hour work week, typically 9:00 AM-6:00 PM Eastern Time, Monday through Friday.
Because the Clinical Lead RN serves as a resource for Care Coaches and patients, consistent attendance, reliability, and availability during scheduled working hours are essential.
Compensation
Starting Pay: $25-$33 per hour, based on experience and qualifications.
This is a full-time position and includes paid time off, holiday pay, and access to our benefits package.

Pay: $25.00 - $33.00 per hour
